Pediatric physical therapy

We also offer comprehensive pediatric rehab services through The Children's Hospital at TriStar Centennial. Your child will receive a customized plan for therapy in a comfortable outpatient setting. Our dedicated pediatric therapists have extensive experience and special training to provide care for a wide range of conditions.

Our sports medicine wellness gym

At the TriStar Centennial Sports Medicine Wellness Gym, an exercise physiologist and registered nurse provide health assessments and exercise prescriptions for a variety of health challenges and limitations.

Referrals and appointments

A physician's referral is required for rehabilitation services. If you believe you may benefit from physical or occupational therapy, talk to your doctor. After a medical evaluation, your physician may determine therapy is appropriate for your condition.

Please fax your physician's referral to (615) 342-5609 or bring it with you to your first appointment.

We are located on the first floor of the Sarah Cannon Cancer Center at TriStar Centennial. We are open Monday through Friday from 7:00am to 5:00pm.
