Horizon announces HCA Healthcare Foundation’s $50,000 to the Dickson Community Clinic

TriStar Horizon has supported the Dickson Community Clinic since its inception in 2006. The clinic's mission to provide access to primary care for the uninsured and underinsured in Dickson and surrounding counties aligns with Horizon's commitment to expanding access to essential healthcare services for the community.

Dickson, Tenn. — TriStar Horizon Medical Center, part of HCA Healthcare, announced today that the HCA Healthcare Foundation has awarded a $50,000 grant to the Dickson Community Clinic, supporting the nonprofit primary clinic serving uninsured patients in Dickson and surrounding counties.

“We are proud to support the Dickson Community Clinic and the incredible work they do for families in our community,” said Wyatt Chocklett, chief executive officer at TriStar Horizon. “Their mission to provide primary care and serve as a medical home for uninsured patients aligns perfectly with our commitment to improving the health and well-being of our neighbors.”

Founded in 2006, the Dickson Community Clinic provides essential healthcare services to patients who otherwise might not have access to a primary provider.  The clinic currently serves as the primary care home for over 2,500 patients, averaging 3,200 patient visits each year.  The clinic is staffed by a licensed nurse practitioner and nurse, and is supported by a volunteer network of specialists, providers, and staff. 

“This grant is critically important to the operation of the clinic,” said Dr. Jeffrey Gordon, past president of the Dickson Community Clinic board of directors. “With this support, we have the resources to provide affordable healthcare for the members of our community who may otherwise not have access to a primary care provider.” 

The funding comes from the HCA Healthcare Foundation’s Middle Tennessee Fund, which supports organizations across Davidson, Williamson, Wilson, Rutherford, Cheatham, Dickson, Robertson, and Sumner counties. Since its creation in 1997, the fund has prioritized investments in organizations addressing basic needs, health and well-being, primary care, education, and the arts. The fund places strong emphasis on nonprofits that maintain authentic, ongoing relationships with HCA Healthcare colleagues, often demonstrated through board service and volunteerism.  In 2025, the HCA Healthcare Foundation’s Middle Tennessee Fund awarded 261 grants totaling $7,515,000 to 221 nonprofits.

TriStar Horizon has supported the Dickson County Community Clinic since its inception, with TriStar Horizon providers and nursing colleagues volunteering their time and expertise for nearly two decades.  This collaboration reflects the shared goal of creating healthier tomorrows by expanding access to essential healthcare services for the community.
